Carpal tunnel syndrome (CTS) is a condition caused by compression of the median nerve as it passes through the wrist that’s characterized by pain, numbness, tingling, and weakness in select parts of the hand. The result of a 2019 study suggests that CTS may be associated with migraine headaches. How can these two seemingly disparate conditions be related?

The study itself analyzed data gathered from a survey of nearly 26,000 adults in the United States. In particular, the researchers looked at responses to questions about CTS-related symptoms within the previous year and whether or not respondents had a migraine in the past three months. The results showed that 3.7% of participants had CTS symptoms in the last twelve months and 16.3% had a migraine in the last 90 days.

When the research team compared those with CTS symptoms and those with migraines, they found that roughly a third (34%) of CTS-positive respondents also reported migraines, which is about 2.6 times more than study participants who did not experience CTS-related symptoms. On the other hand, 8% of individuals in the migraine group had CTS-associated symptoms in the last year compared with just 3% in the non-migraine group.

The relationship between the two conditions indicates they may share one or more underlying cause. One such cause may be cervical dysfunction. While CTS is generally described as a condition caused by compression of the median nerve at the wrist, pinching of the median nerve anywhere along its course from the neck to the wrist can generate hand/wrist symptoms. In many cases, a patient complaining of such symptoms may have median nerve entrapment in several areas that all need to be treated to achieve a successful outcome. While there are many potential triggers for a migraine episode, the root cause/s of the condition are not well understood. However, several studies have found that patients with migraines are more likely to have trigger points in their neck muscles and other cervical issues, and treatment to improve function in this area can reduce the intensity and frequency of migraines.

With both conditions, it’s important to undergo a thorough examination to identify any and all contributing factors—including cervical dysfunction. Carpal tunnel syndrome (CTS) is the most common and well-known nerve entrapment condition, often called neuropathy, affecting and causing symptoms in your entire shoulder girdle, arms, hands, and fingers. It’s basically a pinched nerve in your extremities somewhere between your neck and hand. Often people think that carpal tunnel syndrome (CTS) affects only the all day prolonged computer or desk worker yet that’s far from reality.

Though Carpal tunnel syndrome symptoms can affect anyone, does it surprise you that it’s quite common in some of the most active people, such as athletes? Let’s look a little closer at the reasons for why carpal tunnel symptoms are so prevalent in even who we consider to be the most elite fit athletes…

Have you noticed how quite a few sports, such as baseball, cycling, tennis, and golf, require you to grip tightly for long intervals without any rest? This greatly fatigues the neck, shoulder, and forearm muscles, tendons, ligaments over time, stressing the joints, and creating muscle imbalances. These muscle imbalances create rounded shoulders and forward head posture, often put press on nerves as well as create pain landmine knots called “trigger points.” Ultimately, you’re left with pain and numbness, even tingling in your hands, arms, fingers. Carpal tunnel syndrome (CTS) is a common condition that affects around 6-12% of the population and can result in significant pain and disability. The financial costs associated with CTS can be staggering – ranging from $45,000 to $89,000 per patient over a six-year period when productivity loses are taken into account.

Historically, doctors and researchers have described CTS as the result of compression of the median nerve as it travels through the bony carpal tunnel at the wrist. However, there is recent evidence that CTS is a more complex pain syndrome with multiple studies showing women with CTS exhibit widespread pressure pain hypersensitivity, thermal pain increases, and what’s called “enhanced wind-up in extra-median nerve territories.” In other words, the central nervous system seems to be involved, affecting the whole body, not just the wrist and hand.

Traditionally, the management of CTS has included conservative interventions primarily focused on relieving wrist and hand symptoms using splints, manual therapies, modalities (ultrasound, laser), and exercise—with surgery recommended if the patient fails to respond treatment. In looking at CTS as a product of the central nervous system (CNS), therapies that target desensitizing the nervous system may be more effective.

A 2017 randomized clinical trial compared manual therapy with surgery for improving BOTH pain and central sensitization (“nociceptive gain”) in CTS patients. Here, researchers randomly assigned 100 women to either a manual therapy group who received one session per week for three weeks including “desensitizing manoeuvres of the CNS” or a surgical intervention group (50 in each group). The research team evaluated pressure pain thresholds (PPT), thermal pain thresholds (hot or cold – HPT or CPT), and pain intensity at baseline, three, six, nine, and twelve month intervals following the intervention.

After one year, those in the manual therapy group experienced higher increases in PPT over the carpal tunnel at three, six, and nine months and greater decreases in pain intensity at three-months than those who underwent a surgical procedure. Otherwise, the outcome measurements were similar for both groups.

The significance of this study supports that a non-surgical, manual therapy approach (in which neuromuscular/trigger point massage specializes) is more effective in the short term and equally effective in the long term as surgery for BOTH pain and central sensitization (PPT only – not HPT/CPT for either group).
